Our specialty

We specialise in
complex care

When support needs are complex, one worker and one plan is rarely enough. Mindhaven Care brings together nurses, mental health professionals, behaviour support practitioners and allied health professionals in one team — so clinical, behavioural and everyday supports finally pull in the same direction.

Our team

One multidisciplinary team around the participant

Complex care works when clinical and everyday supports talk to each other. Our team holds the whole picture, so nothing falls between providers.

Nurses

Clinical oversight for health-related and high-intensity supports, staff competency and safe support planning.

Mental health professionals

Support for psychosocial disability, emotional regulation, wellbeing and recovery-focused planning.

Behaviour support practitioners

Functional assessments, behaviour support plans and strategies that reduce restrictive practices.

Allied health professionals

Practical input across communication, function, mobility and daily living skills.

Who we support

Built for the referrals others find hard

We welcome complex referrals — including participants whose supports have escalated or broken down elsewhere. Our intake looks at clinical risk, behaviour and environment together before we commit to a plan.

  • Participants with behaviours of concern or restrictive practices in place
  • High-intensity daily living and health-related support needs
  • Psychosocial disability and dual diagnosis
  • Autism, intellectual disability and acquired brain injury with complex presentations
  • Children and young people with disability, including physical disability
  • Acquired Brain Injury (ABI)
  • Spinal cord injury
  • Cerebral palsy
  • Chronic and complex health conditions
  • Participants transitioning from hospital, SIL or crisis accommodation
  • Situations where previous supports have broken down or escalated

How complex care works

Clinical thinking behind everyday support

01

Clinical intake

We gather the full picture — health, behaviour, mental health, environment and risk — before support begins.

02

One shared plan

Our nurses, mental health clinicians, behaviour support practitioners and allied health team write one coordinated plan.

03

Highly trained support workers

Our support workers are highly trained against that plan and clinically supervised, so strategies are delivered consistently and safely.

04

Review and adjust

We track how supports are working and adjust as needs change, keeping the participant and their team informed.
